How do I add a custom 404 error page?

A reader writes:
Your intuitive.com site (which I love, by the way! great site!) has a custom error page when I enter a bad URL. I've seen that elsewhere too, but don't really know how to do it. Any hints?

Dave's Answer: Better than a hint, I've built an entire Web site talking about how to do this, why to do this, and some fun ideas to make your error page maximally useful. Where is this site, you ask?

Check it out (and pay attention to what's really there, not what it looks like!) and let me know what you think!

I have something to say, now that you mention it, but ...
Starbucks coffee cup I do have a lot to say, and questions of my own for that matter, but first I'd like to say thank you for all your efforts on this Web site by buying you a cup of coffee!

I do have a comment, now that you mention it!
